2) Complete the forms. Please answer every question.
3) Once forms are complete

a) Fax to 317-577-7538 (you will not need to arrive prior to your scheduled appointment time)

b) Bring completed forms with you to your appointment (please arrive 15 minutes prior to your appointment time).
